Sick Notes / Fit Notes

FIRST WEEK - For the first week of sickness you do not need a sicknote; you can certify your sickness using a form called SC2 available from your employer, online or the benefits agency. 

AFTER THE FIRST WEEK - You will need a sick note.  Please telephone the Reception Team to discuss this further, you may need to make an appointment to see one of our Clinicians. Please note that sick notes CANNOT be forward dated.

PRIVATE SICK NOTES - Unfortunately some employers insist on a sick note from a Doctor even if you have been off work for less than 1 week.  Providing a sick-note under these circumstances is not a legal requirement, and is not supported by the NHS.  These medical notes take Clinical time away from providing valuable NHS services, and as a result we have to charge for issuing private sick notes.

Test results

The Surgery receives numerous test results each day and we have a robust monitoring system.  The Surgery will not routinely inform you of a result.  If you have had a blood test and wish to know the result, please all 10 days before contacting the Surgery and telephone between 11am – 1pm.

For ALL X-Ray results we ask that you make an appointment with the requesting GP for the result.

To ensure confidentiality, we only release results to the patient, unless alternative arrangements have been agreed in writing.

 

Non-NHS services

The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. Our fees for these services, which are in line with BMA approved national guidelines, are displayed in the Surgery, or available upon request.
